    In Sjögren’s, Plaquenil is used to treat many symptoms of Sjögren’s including fatigue, joint symptoms of arthritis and arthralgias (joint pain), dry mouth and dry eyes. Similar to its use in systemic lupus erythematosus, many clinicians feel that it is useful in reducing general Sjögren’s “disease activity.” One of the reasons that physicians feel comfortable in prescribing Plaquenil is its low risk to benefit ratio.

    The changes are most often permanent, but in some cases, vision has improved. Most rheumatologists recommend patients undergo a baseline eye examination prior to starting Plaquenil and be re-examined in the future depending on their risk for developing the condition. Risk factors include advanced age and having pre-existing retinal disease. All individuals starting Plaquenil treatment should have a baseline ophthalmologic examination within the first year. If the initial ophthalmologic examination is normal and the person is considered low risk, re-testing need not occur for five years. It is recommended that high-risk patients have annual eye examinations.   Abnormal test is the visual field test, you may be asked to repeat it to make sure of the results. You will be told if any other tests are necessary and when you should be screened again. In cases of possible retinopathy, you will be recommended to continue taking 8 9 Macular Society Eye screening for patients taking hydroxychloroquine

    As with any medication, allergic reactions including skin rashes and non-allergic reactions can occur. This means that the side effects of Plaquenil are mild and infrequent compared with its potential benefits.

  5. My Scleroderma/morphea is spreading above my eye so I am pretty urgently looking to get on Plaquenil but obviously don't want to potentially screw up my eyesight! Does this sound right, starting the Plaquenil before a baseline test can be done? I made an appointment with my family doctor next week so that I can discuss it with her.

    Baseline Examination. All patients beginning long-term HCQ or CQ therapy should have a baseline ophthalmologic examination within the first year of starting the drug to document any complicating ocular conditions and to establish a record of the fundus appearance and functional status. Remember, any patient who is at a high risk for Plaquenil toxicity should be examined on an annual basis following the baseline exam.
